Who’s information should be on the form?

The account holder should fill out the form. There should only be one account holder per household.

 

Why do I need to provide this information?

A 2012 federal law requires recertification of eligibility and that the household has only one Lifeline account ever year.

 

Why did my phone bill go up?

You may need to recertify. After receiving notification that you need to recertify from the phone company by mail, you will have 30 days to respond.
